Tesamorelin is a stabilized analog of human growth-hormone-releasing hormone built on the full 1-44 sequence, distinguished by an N-terminal trans-3-hexenoyl group that protects the peptide from rapid enzymatic degradation. That single acyl cap is the defining structural feature that separates it from unmodified GHRH.
The acyl cap
Attaching a hexenoyl moiety at the N-terminus shields the vulnerable amino end from aminopeptidase cleavage, extending in-vitro stability while preserving the receptor engagement described in its mechanism of action. Because degradation of native GHRH frequently begins at the N-terminus, capping that position is a targeted way to slow breakdown.
Full-length GHRH backbone
Unlike the truncated sermorelin(1-29), tesamorelin retains the longer 44-residue sequence. Preserving the full backbone maintains the native folding context but also increases synthesis complexity, since more residues means more coupling cycles and greater chance of accumulated byproducts.
Synthesis considerations
The longer chain places extra demands on coupling efficiency and purification. Each additional residue raises the risk of deletion sequences, so high-quality preparation depends on optimized coupling chemistry and careful chromatographic cleanup to isolate the correct product.
Build and QC

Comparison with other stabilization strategies
N-terminal acylation is only one way to stabilize a GHRH analog. It differs structurally from substituting protease-sensitive residues in a shortened fragment and from adding an albumin-reactive affinity complex. Tesamorelin's design retains the full backbone while protecting a vulnerable terminus, so comparisons with other analogs should distinguish chain length, local sequence substitutions, and carrier binding rather than treating all extended stability as chemically equivalent.
